1. Declaration Form


This form is a formal statement made by a health professional affirming their membership in a recognized institution or professional body and confirming that they are qualified to practice a specific Allied Health Profession. It must be signed in the presence of a Commissioner of Oaths, making it a legally binding declaration used as part of the licensing process by the Allied Health Professions Council (AHPC) in Guyana.


2. Annual Licence to Practice


This certificate is issued by the AHPC to an individual who has been officially registered and is authorized to practice as an Allied Health Professional in Guyana. It confirms that the professional has met all regulatory requirements and is licensed to practice within a specified timeframe—typically for one calendar year.


3. Application for Annual Licence to Practice


This is the official form used by health professionals to apply for or renew their license to practice in Guyana. It collects essential information such as contact details, work designation, type of practice, and continuing education (CE) credits. The form must be completed and submitted annually to maintain licensure.


4. Applying for CE Credits


This form is used by health professionals to request Continuing Education (CE) credits for various learning activities, such as attending training sessions, conducting research, or participating in workshops. CE credits are essential for the annual renewal of licenses and ensure that practitioners stay updated in their field.
